5 Questions to Ask Before You Invest in a Theranostics Program

Many radiation oncology programs are considering adding theranostics to their services to expand patient access, strengthen referral relationships, and position themselves for the future. But before making the investment, cancer center leaders should take a step back and ask themselves a few questions to help evaluate whether their program is truly prepared.

Here are five questions you should ask before jumping in:

1. Does a Theranostics Program Make Sense for Our Patients?

Adding theranostics feels exciting, but it takes more than excitement to build a sustainable program. Start by evaluating your patient population, referral patterns, and market opportunities.

Many programs assume that offering theranostics will automatically attract new patients.  But “build it, and they will come” is not a strategy. In reality, successful programs are built on a steady stream of eligible candidates and strong referral relationships. A center that only treats a handful of qualifying patients each year will struggle to justify the operational and financial resources needed to support the program. It’s worth it to take the time to study your market carefully before you take the leap.

2. Can Our Referral Network Support a Theranostics Program?

A theranostics program requires collaboration!  Radiation oncologists, medical oncologists, urologists, and other specialists all play a role in identifying appropriate patients. Those specialists need to understand which patients may benefit from treatment and the value the program will bring to their patients.  Do you have those relationships, and are you prepared to provide the necessary information and education?

It’s also important to assess the competitive landscape. Are patients going to another center for theranostics now? Are there community physicians who would rather keep their patients closer to home if these services were available locally? Understand these dynamics to help determine if your program will fill a genuine need.

3. Can We Manage the Financial Risks of Theranostics?

Many radiopharmaceutical therapies have significant acquisition costs. Reimbursement can be high, but often requires prior authorization, detailed documentation, and a plan for denial management.

If your organization is considering theranostics, be sure you have a clear understanding of the possible cash flow implications, reimbursement process, and revenue cycle management needs before you launch.

4. Are We Operationally Ready?

Offering theranostics involves more than administering the treatment.

Radiation safety protocols, staff training, scheduling workflows, patient documentation, billing, and regulatory compliance must all be in place. The centers that struggle with theranostics are often those that underestimate the operational planning required.

5. Are We Building a Theranostics Program, or Just Adding a Treatment?

The most successful organizations view theranostics as a strategic service line. They develop the infrastructure, relationships, and long-term vision to support growth, and are ready to scale it as new radiopharmaceutical therapies enter the market.

Ask yourself honestly if you are just responding to industry buzz, or if you are ready to create a sustainable program that aligns with your organization’s goals.

Theranostics may be one of the most significant opportunities in radiation oncology today.  It has the potential to expand treatment options, elevate your market position, and create new opportunities for growth. But it requires careful planning, strong physician partnerships, operational readiness, and a clear financial strategy.

At Radiation Business Solutions, we work with cancer centers every day to help them strategically incorporate new technologies and new service lines. If you’re exploring whether or not a theranostics program is right for your organization, we’d be happy to help you analyze the opportunities, risks, and practical considerations before you make the investment.
